Academic forms that are used with students are now available in a single location on the MyCanisius portal. Here’s how to access everything from “incompletes” to “internships”:
- Under Online Tools, select the EMAIL, D2L, & Services tab (click on the + sign)
- Open “All Applications & Services” (click on the bottom most tab)
- The screen will open with all the academic forms in the right most box under the FORMS heading
Please discard previous versions of these forms and use those found on MyCanisius. Future updates will be saved to this location, to ensure that the most current forms are available for your use.
