Open enrollment for Canisius Care begins today (November 14) and runs through December 9. Anyone wishing to enroll for 2012 must complete an enrollment form and return the form to the Office of Human Resources. Enrollment forms can be found here.
Current participants in the plan should not discard their PowerPay card from Independent Health. The card will be reloaded at the end of 2011 with the amount elected for 2012. An enrollment form must be completed.
For those who do not wish to re-enroll, please Email Dawn Rotterman at rottermd@canisius.edu stating that you are declining for 2012.
