Open enrollment for health insurance ends on June 15. Information regarding changes to co-pays and contribution costs may be viewed on the HR web site. All changes take effect on July 1. Reminder – this is the last year the PPO plan will be open for new enrollees.
If you are not making a change, you do not need to complete any paperwork. You will receive new identification cards prior to July 1.
If you wish to make a change, you need to complete an enrollment form and return it to Human Resources by Friday, June 15.
