Anyone wishing to enroll for 2013 must complete an enrollment form and return the form to Human Resources. Enrollment forms can be found at http://www.canisius.edu/hr/forms/ or in the Human Resources Office. Current participants MUST complete an enrollment form for 2013.
If you do not wish to re-enroll, please email Dawn Rotterman at rottermd@canisius.edu stating that you are declining for 2013.
